All Smart Program Volunteers are required to complete the Accident Prevention Training during their initial training and OJT and annually thereafter.

Download the quiz sheet naming and saving it as its own file to your computer. Open in .pdf and you should see the blue fields allowing you to insert your answers. Once your quiz is completed, save it once again to your computer. Attach your quiz to an email to your Trainer/Director for grading and to update your training chart.
